UNION COUNTY PRIMARY SCHOOL 2010 - 2011
KINDERGARTEN REGISTRATION

Kindergarten screening and pre-registration for the upcoming 2010-2011 school year will be Monday, March 29th through Friday, April 2nd at Union County Primary School. Students will be registered by appointment from 9:00 AM to 11:40 AM and 12:40 PM to 2:00 PM. Parents may come by or call the Primary School at 706-745-5450 to schedule a registration appointment. Please schedule an appointment as early as possible for the best selection in appointment times.


Pre-registration is required and an enrollment packet is available on the school system’s website at www.ucschools.org or at the Primary School’s front office. It is highly recommended that parents download or pick-up a registration packet and complete the forms prior to the scheduled appointment in order to expedite the registration process.


All children who turn five years of age on or before September 1st, 2010 are eligible to attend kindergarten in the upcoming school year. Eligible children who have been attending Pre-K, the Child Enrichment Center, or any other Day Care program will need to be pre-registered for kindergarten.
Parents and students will need to report to the Primary School lobby area for their pre-registration appointment and bring the following items.

 

  • Parent’s Photo I.D.
  • Completed Enrollment Packet
  • Certified copy of student’s Birth Certificate; or certified hospital issued birth record or birth certificate
  • Student’s Social Security Card
  • Immunization Certificate
    -- Current Georgia Department of Human Resources immunization record (Form 3231)
    -- Note: Out of State Immunization Records will need to be taken to your GA primary physician or Union
    County Health Department to be transferred to a Georgia Immunization Certificate (Form 3231)
  • Proof of Union County residency
    -- Utility bill reflecting address,
    -- Driver’s license with correct address,
    -- Automobile Tag Receipt and/or,
    -- Voter’s Registration Card

REGISTRO AL JARDIN DE LA INFANCIA

La evaluación y el pre-registro para el Jardín de la Infancia (Kindergarten) para el próximo año escolar 2010-2011 serán desde el lunes,  29 de marzo hasta el viernes 2 de abril en el Union County Primary School. Los estudiantes serán registrados (matriculados) mediante cita, de 9:00 am a 11:40 am, y de 12:40 pm. a 2:00 pm. Los padres pueden venir o llamar a la Escuela Primaria al 706-745-5450 para pedir una cita de matrícula. Por favor pida una cita lo más pronto posible para escoger la mejor hora de su cita.

Es mandatorio un pre-registro y el paquete de documentos para la matricula está disponible en el sitio web del sistema escolar en www.ucschools.org , o en la oficina principal de la Escuela Primaria. Es recomendable que los padres descarguen del sitio web de la escuela o recojan el paquete de los documentoss para la matrícula y llenen los formatos antes de la cita planificada para asi facilitar el proceso de matrícula/registro.

Todos los niños que cumplen cinco años de edad en o antes del 1ro. de setiembre, 2010 tienen derecho a asistir al jardín de la infancia el próximo año escolar. Los niños elegibles que han estado asistiendo al Pre-K, al Child Enrichment Center (Centro de Enriquecimiento del Niño), o a cualquier otro programa de Cuidado de Guardería Infantil, tienen que ser pre-registrado en el jardín de la infancia

Los padres y los estudiantes tienen que venir al área del vestíbulo de la Escuela Primaria el día de su cita de pre-registro y traer los siguientes documentos:

  • Tarjeta de Identificación del Padre con foto
  • El paquete de documentos para la matrícula completamente llenado
  • Copia del Acta o Certificado de Nacimiento del estudiante; or certificado de nacimiento emitido por el hospital, o Acta o Certificado de Nacimiento
  • Tarjeta del Seguro Social del estudiante
  • Certificado de Vacunas
    - - Certificado actualizado de Vacunas del Departamento de Recursos Humanos (Formato 3231)
    - - Nota: El Certificado de Vacunas de otro estado tiene que ser llevado a su doctor en Georgia, o al Departamento de Salud del Condado Union para ser transferido al Certificado de Vacunas de Georgia  (Formato 3231)
  • Prueba de Residencia en el Condado Union
    -Factura de la electricidad que refleje el domicilio,
    -Licencia de conducir con el domicilio correcto,
    -Recibo de la matrícula/placa del automóvil, y/o
    -Tarjeta de registro de votación electoral
