When enrolling your child in our school, you must bring the following:

• The child's up-to-date Georgia Immunization Records (any student entering 6th grade must make sure that he/she has had 2 MMR Immunization shots before registering

• A copy of his/her Birth Certificate and his/her Social Security Card showing their ID number

• Proof of the Parent/Guardian's physical address in Union County

• A copy of the student's last report card

If your child has an out-of-state immunization record, you will need to have it converted to a Georgia form at the Union County Health Department PRIOR to registering. You may reach the Health Department at (706) 745-6292 for immunization details. If applicable, parents and/or guardians will also need to provide the school with copies of legal arrangements that might affect your child and the school. Academic records from the last school that your child attended will be requested by mail, so the previous school's address and telephone number will also be needed at the time of registration.
